Model Release Form

By signing this release, not only do I confirm that I am over the age of eighteen (18) years, but that I also provide consent to photographer/filmmaker Dan Valentine of Val Entertainment (video & photographic services) to photograph and/or video record me this day and use my image or any video or photographic production, or part thereof, as reference material to promote Val Entertainment on any internet based web site associated with Val Entertainment, including Flickr, Model Mayhem, Vimeo and Youtube, with the condition that I first have a viewing of the material produced for approval purposes and that no unapproved image of me will be published by Val Entertainment.

I also provide permission for Dan Valentine to modify the produced material as necessary for the purposes of promoting Val Entertainment, and that all footage, raw or edited, remains solely the property of Val Entertainment as does any copyright, except where the exemption below is signed by Dan Valentine of Val Entertainment. Where the exemption is signed, I accept that any image that is used by me for any commercial purpose shall be given due credit to Dan Valentine as the image maker and that any image modification will be approved by Dan Valentine before being published in any fashion.

Exemption to Ownership & Copyright of Produced Material

By signing this exemption, I consent to the above named model having equal ownership rights and copyright to the material produced by me on this occasion, but that it does not take away my rights to use the material as already mentioned.
